Build and publish forms — no code required.
What it is
FormBuilder is a visual editor for building forms from text, choice, date, signature, file-upload, and display fields, organizing them into pages, and publishing them at a shareable link. Responses land in a submissions inbox with search, filtering, archiving, and CSV/PDF export where the account is entitled to it.
Security controls
Public submissions are bot-protected, uploads are type-validated, and access to responses is controlled per form. Encryption is selected per form, with a CUI option for work under a DFARS 252.204-7012 contract.
Multilingual forms
Forms can be offered in multiple languages with per-field translations, automatic translation assistance, and a language picker for respondents.
Signatures and documents
DocuBuilder handles multi-party signature envelopes with an audit trail and a certificate of completion. Signature fields can also be embedded directly in forms.

Your forms and the responses you collect are your content and your responsibility — including the lawfulness of what you collect and the consents you obtain from respondents. Use of the Service is governed by our Terms of Use and Privacy Policy.
